One record of the closure itself. It holds the date and time, the reason you chose, anything you wrote in the box, and the IP address the request came from. It holds no username, no email address and no name.
The reason and the comment are how we learn what to fix. The IP address is there for a narrower purpose: it is what distinguishes a genuine run of people leaving from one address repeating itself, which is a security question rather than a product one.
Separately, if you ever reported a problem with a specific question, that report stays, because it is about the question rather than about you. Its link to your account is removed when the account goes, so it no longer names anyone.
The web server keeps its own access logs, as every website does. They are the hosting provider's, not part of the app, and they rotate on the provider's schedule.
Uninstalling Bastion Prep removes the app from your phone and leaves the account untouched, because your history lives on the server so that it follows you between the app and the web. Equally, deleting your account does not uninstall the app: it will simply have nothing to sign in to.
